Insurance Coverage

Dr. Collier is an “In Network” surgical provider for most major insurance companies. However, each insurance company has established criteria for approval of weight loss surgery that must be completed by each patient prior to approval, or preauthorization, for your surgery. If your insurance covers the LAP-BAND procedure, you will be responsible for deductible and co-insurance amount. Our staff is more than happy to help you calculate that amount.

Our staff will work closely with you and assist you in providing necessary information to successfully obtain approval for your surgery as soon as possible. This may require a letter from a medical specialist, if deemed necessary after your evaluation. A letter of medical necessity from your primary care physician regarding your obesity and recommendation for weight loss surgery may also be required.

Private Payment

You may, of course, pay by cash, cashier's check or major credit card the day of surgery.

Third Party Financing

There are several medical financial lending providers who will seek to assist you in obtaining assistance for financial coverage for this elective surgical procedure.
